Starting Friday, a specific form is needed for vulnerable people under 65 to receive the vaccine, FEMA says

MIAMI, Fla. – Some people who showed up on Thursday with doctor’s notes could get a vaccination at the state-run FEMA sites, but they will be turned down starting Friday.

Gov. Ron DeSantis expanded the suitability of the COVID-19 vaccine in Florida on Friday by signing an executive order that now allows doctors to determine who is extremely vulnerable to COVID-19.

Under the addition of Governor Ron DeSantis to the first phase of vaccination plans for Floridians, Executive Order 21-46 states that individuals who are considered by a physician to have a high risk medical condition can now receive the COVID-19 vaccine.

The Florida Department of Health released a copy of the form a doctor must use to certify that someone is extremely vulnerable to the virus and is a candidate for the vaccine via Twitter. On Friday, a rule will come into effect that only those with the correct forms can get the vaccine. A doctor’s note or prescription from a doctor’s pillow is also not acceptable.

“Sure, the last thing we wanted (Thursday) is for someone to waste a trip and not get service and keep any inconvenience to a minimum. At the end of the day, we have vaccines and we want to get them out, ”said Mike Jachles, FEMA’s Master Public Information Officer.

But Jachles said that as of Friday, nothing will be accepted other than the form signed by a doctor. The form is intended for individuals under the age of 65 who meet the extreme vulnerability criteria set by a physician.

The state-run sites of Marlins Park and Hard Rock Stadium will not administer vaccines to high-risk individuals under the age of 65. Those recipients are only accepted on the FEMA sites. FEMA is fully financing the temporary site, which will be open for eight weeks. Miami Dade College’s North Campus has a FEMA location where they can administer 3,000 COVID-19 shots per day.

The site is open every day from 7am to 7pm and no appointments are required.

FEMA also supports two satellite hub vaccination centers. They are located in Sweetwater at Ronseli Park (250 Southwest 114th Avenue) and Florida City at the Youth Activity Center (650 Northwest 5th Avenue). Both offer 500 doses daily and are open seven days a week from 9:00 am to 5:00 pm.

People at risk for serious illness from COVID-19 have conditions such as cancer, COPD, heart disease, obesity, sickle cell disease, type 2 diabetes, and more, according to the Centers for Disease Control and Prevention.
