The couple share eight-year-old Camden Jack Cutler, six-year-old Jaxon Wyatt Cutler and five-year-old Saylor James Cutler.
Kristin and Jay announced their intention to divorce in April after seven years of marriage and 10 years together.
She asked to be changed back to her maiden name on November 4, 2020, and the process was completed the following day, according to reports in US Weekly.
