50 vials of Moderna vaccine are thrown away at Grafton clinic for ‘human error,’ says lawyer Aurora

Dozens of vials of the Moderna COVID-19 vaccine had to be discarded because of “human error,” Aurora Health Care officials said.

Attorney Aurora Health officials said the incident occurred over the weekend, when an employee took the bottles out of the Grafton clinic’s refrigerator to access something else and did not return them.

“We have learned that about 50 vials of the Moderna vaccine were accidentally removed from a pharmacy refrigerator overnight,” said a spokesman. “Although some of the vaccine was administered to team members on December 26 within the approved 12-hour period after refrigeration, unfortunately most of it had to be discarded due to the temperature storage required to maintain viability.”

According to the health service, 17,000 team members have been vaccinated in the past 12 days.

“We are clearly disappointed and regret that this has happened,” said a spokesman.
